Please fill out the form below for either Part-time or Full-time employee:<\/em><\/p>\n In addition<\/strong> to filling out the form above, please make your address change with your Health Benefits:<\/em><\/p>\n The New Jersey Earned Sick Leave Law extends earned sick leave benefits to employees who were not previously entitled to this benefit (i.e. adjuncts, temporary, hourly employees, and student workers).<\/p>\n Under the New Jersey Earned Sick Leave Law, eligible employees earn sick leave at the rate of one-hour for every 30 hours worked, up to a maximum of 40 hours of sick leave.<\/p>\n Eligible employees will begin to accrue earned sick leave on October 29, 2018, or on their first day of employment, whichever is later.<\/p>\n Eligible employees can begin using earned sick leave accrued under the New Jersey Earned Sick Leave Law the later<\/strong> of the 120th<\/sup> day after the accrual date of October 29, 2018 on and after February 26, 2019 or the 120th calendar day after you begin employment.<\/p>\n If your need for earned sick leave is foreseeable (can be planned in advance), you should as possible provide up to 7 days advance notice of your intention to use earned sick leave to your supervisor. If your need for earned sick leave is unforeseeable (cannot be planned in advance), provide notice as soon as it is practical to your supervisor.<\/p>\n Documentation is required, if you use earned sick leave on 3 or more consecutive work days, or on certain Blackout dates specified by your supervisor. However, the law prohibits employers from requiring your health care provider to specify the medical reason for your leave.<\/p>\n Yes, you may carry-over up to 40 hours of leave per benefit year. However, you can only use up to 40 hours of leave per benefit year.<\/p>\n The law recognizes the following individuals as \u201cfamily members:\u201d<\/p>\n\n
